The Hoffman Wellness Program respects your personal privacy and is committed to adhering to federal and state privacy laws to protect you and your personal health information and identity. The OHSU wellness staff and your health plan provider will know the results of your biometric test and quality health survey, however, this same information will NOT be shared with your employer, supervisors, or anyone else at Hoffman. All personal health data will be kept separate, secure and in a confidential database by OHSU wellness and your health plan provider. Hoffman Benefits will receive aggregate or group summary format information and will exclude any employee or spouse names or other identifying information.

*Only exception to absolute confidentiality is if a physical threat is made to self or others.
